Shoulder Pain at Night: Why It Is Worse and What It Means

You have not slept on that side in weeks. You settle, the shoulder starts up, you roll the other way, and it is still going at three in the morning. Lost sleep is what finally sends most people to get a shoulder looked at. The nights are also information.

Why nights are genuinely worse

Three things change when you lie down. The first is load. Lying down changes how the shoulder is stacked, and in some positions the space the cuff tendons pass through under the bony roof of the joint gets tighter, so a tendon that coped all day spends seven hours squeezed.

The second is that nothing else competes for your attention. At two in the morning the pain has the room to itself. The third is the plainest: lying on that side puts your body weight through the sore joint.

Night pain is one of the most ordinary features of a shoulder problem. On its own it is a clue, not an alarm.

What the pattern suggests

  • Rotator cuff trouble. The classic sleep disturber. A deep ache down the outside of the upper arm, worse lying on that side, sore reaching overhead or behind your back. The shoulder still moves when someone else moves it for you.
  • Frozen shoulder. Night pain plus stiffness going global. The arm will not go out to the side or rotate outward even when another person moves it, and it usually started with no injury behind it.
  • Arthritis in the shoulder or the small joint on top of it. Aches after a heavy day of use and stiffens, sometimes grinds.
  • The neck. The lower neck refers pain into the top of the shoulder and down the arm, and does it while the neck itself feels fine. Tingling into the hand, pain past the elbow, or pain that shifts with how your head is turned all point that way. Treating the wrong area is the common error here: a shoulder gets worked on for months while the source sits above it. See neck pain.

Which of those you have is an exam question. The conditions themselves are on frozen shoulder and rotator cuff, the tear question on tear or just soreness, the shoulder broadly on our shoulder pain page.

Getting through tonight

Positioning will not fix the cause. It buys back sleep while you sort the cause out.

  • Lie on the good side and give the sore arm a pillow. A pillow against your chest, the arm resting on it. Unsupported, that arm hangs forward across your body all night.
  • On your back, prop the upper arm. A folded towel under the upper arm, just behind the elbow, keeps it from dropping backwards.
  • In a bad flare, sleep semi-upright. Propped on pillows or in a chair that reclines. For the worst stretch, not for good.
  • Do not sleep with that arm over your head. Common instinct, and it holds the top of the shoulder closed for hours.

General sleep positions are on sleeping position for back and neck pain.

The honest bit about night pain

Pain tied to position, that changes when you shift and lets go once you are up and moving, is a mechanical picture. That is the version that answers to hands-on care.

Pain that does not care what position you are in, wakes you regardless, has no good nights in it, and comes with feeling unwell, losing weight without trying, or fever, is a different pattern. So is pain climbing steadily week over week. That belongs with a physician first.

How Tradewinds sorts it out

At Tradewinds Chiropractic & Wellness in Charlotte Amalie, St. Thomas, the exam compares how far the arm goes on its own against how far it goes when we move it, which separates a stiff shoulder from a painful one. Then cuff strength muscle by muscle, shoulder blade motion, and a neck screen. We ask about the nights directly: which side, whether you wake or cannot drop off, whether any night is better than another. Dr. Keys is the Chair of the Virgin Islands Board of Chiropractic Examiners, a member of the Virgin Islands Chiropractic Society, and the International Chiropractors Association representative for the U.S. Virgin Islands.

Care follows the finding: adjustment of the shoulder, collarbone joints and upper back, soft tissue work, and rehab, all listed on our services page. An arm that is weak rather than sore is an orthopedic question, and we say so.

When you should see someone else

Call 911 for shoulder or arm pain that arrives with pressure or tightness in the chest, breathlessness, a cold sweat, or nausea. That is not a shoulder problem. Women having a heart attack more often feel it as back pain, nausea and shortness of breath than as the crushing chest pain people picture, so "this is not what I imagined a heart attack feels like" is not a safe reason to lie there and wait.

See a physician, not a shoulder exam, for night pain that is unrelenting whatever you do with position, particularly with fever, night sweats, unexplained weight loss, or a history of cancer. A hot, swollen, red shoulder with fever goes to the nearest emergency room. An arm you cannot lift at all after a fall or a hard pull needs an orthopedist promptly.

Questions patients ask

Why does my shoulder hurt more at night?

Three reasons, and all of them are real. Lying down changes how the joint is loaded and narrows the space the cuff tendons pass through, so a tendon that coped during the day gets squeezed for hours. There is nothing else competing for your attention at two in the morning. And lying on that side puts your weight straight through the sore joint. Night pain is a normal feature of shoulder problems rather than a sign of something sinister on its own.

I have shoulder pain at night and can't sleep. What can I do tonight?

Lie on the good side with a pillow against your chest and the sore arm resting on top of it, so the arm is not hanging forward all night. On your back, put a folded towel under the upper arm just behind the elbow. During a bad flare, sleeping propped semi-upright is often easier than lying flat. Avoid sleeping with that arm up over your head, which holds the top of the shoulder closed.

Why can't I sleep on my shoulder?

Because side-lying compresses the joint directly and closes down the space under the bony roof of the shoulder, which is exactly where an irritated rotator cuff tendon sits. Pain lying on that specific side is the classic cuff pattern. A shoulder that is stiff in every direction and will not move even when someone else moves it is pointing more toward frozen shoulder.

Is shoulder pain at night serious?

Usually not by itself, since most shoulder problems disturb sleep. What matters is the pattern. Pain that changes with position and eases once you are up is mechanical. Pain that ignores position entirely, wakes you every night without exception, and comes with fever, night sweats or unexplained weight loss needs a physician rather than a shoulder exam. Shoulder or arm pain with chest pressure, breathlessness, sweating or nausea is a 911 call.

Does night pain mean I tore my rotator cuff?

Not on its own. Irritated tendons, frozen shoulder and arthritis all hurt at night without any tear involved. Night pain that reliably wakes you counts as one sign among several, and weakness matters more: an arm that will not hold out to the side, or that drops when you let go, is the finding that raises real concern. That judgment is covered on our tear or soreness page.
