Why Is My Back So Stiff Every Morning?
/You wake up and your back does not want to move. The first few steps are careful, putting on shoes is the hard part, and then somewhere in the first twenty or thirty minutes it loosens and you get on with your day. The next morning it is back.
That pattern is common and has an ordinary explanation. There is also a version that is not ordinary, and what separates them is how long the stiffness lasts.
What this feels like
- Worst in the first few minutes after standing up, better as you move, mostly gone within half an hour.
- Comes back after any long stretch of sitting, so a long drive brings a smaller version.
- More stiffness than pain. People describe it as rusty or seized rather than sharp.
- No leg symptoms. Pain, numbness or tingling below the knee is a different problem, covered on our sciatica page.
What happens to your back overnight
Three things run at once while you sleep.
Spinal discs behave like sponges. Under the load of a day upright they lose water and height. Lying down takes that load off and the discs draw fluid back in, so by morning they are slightly taller and under more internal pressure. A disc that is already irritated feels that swelling.
Joints that sit still get thick. Facet joints, the small paired joints at the back of each spinal segment, rely on movement to keep their lubricating fluid distributed. Seven or eight hours in one or two positions is a long time without moving through a range.
And swelling does not clear itself out. Inflammatory fluid around an irritated joint is moved along by the mechanical squeeze of walking and bending. Overnight none of that happens, so it pools and the tissue feels tight until movement clears it.
The first twenty to thirty minutes are the worst part, and the loosening is real.
How long the stiffness lasts is the part that matters
Stiffness that lasts a few minutes to half an hour and eases with movement is mechanical. It fits joint and disc wear, an old injury, or long hours in one position, and it responds to the care described below.
Stiffness that runs well past an hour every morning is a different signal. Prolonged morning stiffness is a recognized feature of inflammatory arthritis of the spine, and it usually travels with other findings: pain that wakes you in the second half of the night, other joints that swell, heel or chest wall pain, onset before age 45, or stiffness that improves with exercise but not with rest. That is a rheumatology question, diagnosed with bloodwork and imaging a chiropractor does not order and treated with medication a chiropractor does not prescribe. If it fits you, see a physician about inflammatory back pain.
Mattress and sleep position
They matter, and they are rarely the whole answer. A mattress collapsed in the middle lets your hips sag and holds your low back in a slight sidebend all night, which can turn mild stiffness into a bad morning. Stomach sleeping keeps the low back extended for hours. A pillow between the knees for side sleepers, or under the knees for back sleepers, costs nothing and is worth two weeks before you buy a bed.
What a mattress will not explain is stiffness built over years, or stiffness that shows up in hotel beds and your own bed alike. If you changed the mattress and nothing moved, the problem is the back.
What might be behind it
Morning stiffness is a symptom, not a diagnosis. Most often it comes from disc and facet joint wear (degenerative disc disease) or canal narrowing (spinal stenosis). Those pages describe the conditions. This one describes the complaint, and an exam connects them.
How we treat it at Tradewinds
Tradewinds Chiropractic & Wellness in Charlotte Amalie starts with the duration question above, then tests range of motion, joint movement segment by segment, hip mobility and leg nerve function. Dr. Keys is the Chair of the Virgin Islands Board of Chiropractic Examiners, a member of the Virgin Islands Chiropractic Society, and the International Chiropractors Association representative for the U.S. Virgin Islands.
When the picture is mechanical, care usually combines adjustment or gentle mobilization of the segments that are not moving, soft tissue work for the muscles guarding around them, and a short mobility routine done before you get out of bed. Those methods are on our services page. A realistic goal is a shorter, milder first half hour, not a back that wakes up perfect.
When you should see someone else
Go to the nearest emergency room, or call 911, for loss of bowel or bladder control, numbness in the saddle area, back pain with fever, leg weakness that is getting worse, or back pain after a serious fall or crash.
See a physician first if you have:
- Morning stiffness lasting well over an hour, most mornings
- Pain that wakes you at night and makes you get up and move
- Other joints swollen, warm or stiff, or a family history of inflammatory arthritis
- Unexplained weight loss, or a history of cancer
Visitors, crew and cruise passengers

Questions patients ask
Why is my back so stiff in the morning?
Overnight your discs draw fluid back in and swell slightly, your spinal joints sit still for hours, and there is no movement to pump inflammatory fluid away from irritated tissue. All three peak right when you wake up, which is why the first twenty to thirty minutes are the worst and why moving around loosens it.
Is back stiffness when I wake up serious?
Usually not, if it eases within about half an hour and you have no other joint symptoms. The version worth taking seriously is stiffness that lasts well over an hour every morning, especially with night pain that wakes you or swelling in other joints. That points toward inflammatory arthritis and needs a medical workup, not chiropractic care alone.
I have a stiff lower back every morning that loosens up. What is causing it?
Most often disc and facet joint wear, an old injury, or long hours in one position, all of which stiffen overnight and free up with movement. An exam sorts out which segments are actually restricted. How long the stiffness lasts is the main thing that separates a mechanical cause from an inflammatory one.
Is my mattress causing my morning back stiffness?
It can contribute, especially a mattress that sags in the middle, and stomach sleeping does not help either. It is rarely the whole answer. If you are stiff in your own bed, in hotel beds and after a nap on the couch, the problem is in your back rather than the bedding.
How long should morning back stiffness last before I worry?
Under thirty minutes is the usual mechanical pattern. Consistently over an hour is the threshold clinicians use as a warning sign for inflammatory back pain, particularly when it comes with night pain, other swollen joints, or an onset before age 45. That combination is a reason to see a physician.
